INTERVENTION IN LOGOPEDY COSTS (children)

Welcome to the Fonds Verhelst platform for digital forms!
Please read the following instructions carefully before completing your form.

• This platform allows you to submit and update your application;
•  You can save your application and modify it later at your earliest convenience before submitting it:
  • click on "Save and exit" if you wish to come back to modify it later;
• Read your application carefully before submitting it, make sure that all fields are filled in correctly and that all necessary documents / evidence have been uploaded (if applicable).
• When your application is complete, you can submit it:
  • IMPORTANT! In screen 4 of the form (at the end of the "Summary"), click on "VALIDATE AND SEND". If you do not do this, your application will remain in draft form and cannot be processed
• Once your application has been submitted/sent, it is no longer possible to modify it, unless you receive an update request from us. Your file will then be unlocked to allow you to make the necessary changes. If you would like to make any changes yourself, please contact Annick. Then she can unlock your file.
